Every new patient at Root Therapy and Wellness completes a short intake packet online, before the first visit. The link is sent to you automatically when you book, the packet saves your progress as you go, and you sign once at the end — most people finish in about ten minutes. What Does Intake Cover?
- Contact, insurance, and health history — so we can verify your benefits and your therapist knows your story before you walk in.
- Consent to treat — what therapy involves, its risks, and your right to ask questions and decline any part of treatment at any time.
- Our Notice of Privacy Practices — how your health information is used and protected. You can read it any time.
- Financial responsibility — how insurance billing, copays, and self-pay work, in plain language.
- Our cancellation policy — 24 hours’ notice, and what happens when life gets in the way.
- How you’d like us to communicate — text and email preferences, and who else (a spouse, a grandparent, a caregiver) we may talk to about scheduling or your care.
What Should I Bring?
A photo ID, your insurance card, any imaging reports you have, and a referral if your plan requires one — the full list, plus what to wear and where to park, lives on our New Patient Info page.
Does My Insurance Need Anything First?
Montana is a direct-access state, so most plans don’t require a referral to start therapy — Medicare and Medicaid do, and we’ll help coordinate that with your physician.
